 To reproduce:

 1. Create a new post, select the video post format
 1. Hit Select Video from Media Library
 1. Choose Upload Files and upload an image
 1. Hit Select Video

 An image is inserted instead of a video, plus after saving, the dimensions
 constraints are not applied to the image because it's a video post format.
