 in terms of item 2 i was not able to reproduce the issue with tabbing/tab
 order. in my testing using chrome/firefox on os x i had no problems
 tabbing thru the date and time input fields; also, inspecting the source i
 was not able to find the tabindex values mentioned by the author, perhaps
 they were removed since the ticket was opened?

 the second issue from item 2 is the lack of labels identifying the date
 fields. i can try to add those here in a separate patch.
