 I personally think this is plugin territory.  Just create a checkbox in
 the comment form where a user can "opt-out" of using Gravatar.  Save as
 comment meta, then if the meta is set, just show the default avatar.

 If you don't want your Gravatar shown on sites, then don't use it, or use
 a fake e-mail address.  I don't see the point of this discussion.
