 Seems like some of these need to be more closely evaluated, e.g.

 `<a class="media-modal-close" href="#" title="<?php esc_attr_e('Close');
 ?>"><span class="media-modal-icon"></span></a>`

 I guess the text should be in the span and then pushed off screen /
 otherwise accessibly hidden.
