 [attachment:24046.8.diff] builds on Pete's patch which builds on Mark's.
 The only changes are removing some console.log() calls that seem to have
 snuck into Mark's patch, and fixing the issue Ryan pointed out where the
 add media button looked wrong after it was hidden and re-displayed (it was
 getting set to inline when it should have been inline-block).
