 In [attachment:12285.6.diff]:

 * The original patch changes the Show Toolbar setting output that's in
 trunk currently, reverts to this.
 * In _wp_get_user_namefields(), add a 'description' key to the namefield
 array elements, which outputs in a span after the input.
