Comment:

 [attachment:23964.diff 23964.diff] fixes this.
 `get_the_post_format_image()` now returns a linked image whenever the
 `_wp_format_url` value is un-empty. Image selected via the modal, image
 HTML inserted into the editor, image attached to the post: all work.
