 The text in the installer page reads "In all likelihood, these items were
 supplied to you by your Web Host. If you do not have this information,
 then you will need to contact them before you can continue. If you’re all
 ready…"

 Maybe we can provide a link here to the [http://make.wordpress.org/support
 /user-manual/02-setting-up-wordpress/installing/#create-a-mysql-database-
 and-user make/support page] if that's going to be the canonical support
 info on basic DB setup.
