 Please, no. `masthead` has no clear meaning, this could be almost anywhere
 in the document. How can one target the real start of the document then?

 Plus, the `before` and `after` combinations allow plugin authors to insert
 start and end tags for additional elements (think: reliable CSS
 selectors), something the standalone hooks just cannot do.
