 Actually, you are totally correct, it always happened - I forgot that my
 picture above was just a mockup, not an actual screenshot.

 The only reason it works better in 3.4.2 is that there's a lot of
 whitespace under the permalink, so the wrapping doesn't cover the media
 button. So I opened a new ticket, #22298.
