 From my perspective repeating the buttons visible at the bottom is a good
 idea (even though I haven't seen it yet). It echoes functionality in other
 parts of the admin screens. Also, keyboard accessibility is not just for
 those with poor sight - it's for sighted individuals who can't or have
 difficulty using a mouse. To have to tab all the way back up to the top is
 not great usability. If you really can't have them repeated visibly then
 it might be desirable to just have them at the bottom and not at the top.

 Also, focus arriving on a Close button... this troubles me slightly -
 especially as the alternate text for the close button just says 'Close'.
 Would people using a screen reader get enough context there? Could there
 not be a Heading at the top of the panel that says 'Theme Customizer
 Options' that could get the focus?
