 Media: Add title, caption, and alt text, and attachment info to sidebar.

 * Improve sidebar styles.
 * Separate the hybrid title/caption field.
 * Improve the sidebar image thumbnail.
 * Remove filenames from inside the non-image icon thumbnail.
 * Properly sync title/caption/alt.
