 The Add New Category form in the Categories admin page appears to be
 completely dead. Seemingly, nothing happens - no slug autofill, no
 response to clicking the Add New Category button.

 However, it seems to be working nearly fine. Refresh the Categories screen
 and the new category is in the list.

 I've verified this on two sites that I upgraded to 3.4.
