 In working on the comment markup I realized instead of hacking it to
 pieces with a custom callback passed to `wp_list_comments()` we should eat
 our own dogfood and improve the markup that WP spits out by default.

 The only reason we ''might'' still keep the custom Twenty Twelve comments
 callback is to use HTML5 semantic elements.
