 Steps to Reproduce:

 1. Go to the "Upload New Media" page (wp-admin/media-new.php)
 1. Upload an image.
 1. Create a new post.
 1. Create an image gallery that includes the newly-uploaded image.
 1. Insert the gallery into the post.
 1. Publish the post.

 Expected result:  Any "(Unattached)" image(s) should get attached.

 Actual result:  Images remain "(Unattached)".
