Comment:

 #18708 was asking for the exact opposite, so I would call this a wontfix.
 Maintaining things the way they were before is not necessarily a driving
 factor - better workflows that lend themselves to distinct actions is, and
 in this case setting a featured image really is a distinct action that
 leads to a distinct conclusion. It's unfortunate that we used to mix
 everything up together, because some people have gotten used to it, but we
 need to move on from it. The featured image tab in the new modal was added
 after it was realized that that "Set as featured image" link always showed
 previously, even when the originating button was the one for inserting
 into content, but I think that's as far as is needed to go to support old
 workflows.

 I think, for the moment, it would be best if this was thought of as a
 "media upload, selection, and insertion modal" rather than a manager
 modal.
