 Had a long discussion last night about this. This was my takeaway about
 the outstanding issues:

 * The selection mechanism (click to select/focus, click again to deselect)
 was not very friendly for inserting a single image. We'd inadvertently
 optimized the selection process for the new multi-image insert feature
 (which is a bit of a power user feature). It was too easy to accidentally
 insert multiple images. It was frustrating for the more common use case:
 inserting a single image. By splitting between insert/gallery, we can
 offer a single-image optimized selection pattern for insertion (and let
 cmd/ctrl/shift clicks do multi-image insertion for power users), and offer
 a gallery-optimized selection pattern for gallery creation/addition.
 * It was odd for the sidebar to have a mixture of sources and actions. Now
 it'll be actions. The upload/library choice gets made in the main pane.
 * When thinking about how people would extend this, we feared that "adding
 a new action button to the lower-right" (LTR) would be the approach. There
 wasn't room for it. But there '''is''' room in the sidebar. Lots of room.
 * By moving the screen title inside the lightbox, it becomes more visible
 and more useful. This also let us make the lightbox bigger, which gives
 users more room to see their images.
