 Any luck with this?  For me the issue seems to happen consistently after
 updating the plugin
 "Events Manager"

 The plugins on my MultiSite install that call $wp_rewrite->flush_rules();
 are:

 Events Manager
 Gravity Forms Directory & Addons
 WordPress SEO
