 After a quick chat with @nacin, it seems that most of the
 functions/methods for rendering screen options markup should be emptied
 and deprecated. In their places, introduce a new top-level render method
 with methods to handle each section (show on screen, layout, per page,
 ...). Take the opportunity to continue the screen cleanup.
