 If the default gravatar option is set to "gravatar_default" then
 `get_avatar()` will not send the size parameter to gravatar.com if
 `$email` is empty as [http://core.trac.wordpress.org/browser/trunk/wp-
 includes/pluggable.php?rev=20562#L1649 the question mark separator is
 missing].

 Has been like this since introduction in [7879].
