 There's a weird consistency thing.

 When you're using DFW, the Add Media button is on the bar with B/I/U etc.

 When you're not, it's ABOVE them.  Perhaps we should consider adding it to
 the main toolbar?  And just have the one 'Add Media' button and not the
 trio?
