 Here is a first pass at the UI. Since the admin bar is now always shown in
 the dashboard (#17899), we've focused on making the admin bar contextually
 relevant in both the front end and dashboard. On top of the
 reorganization, we have also merged the admin header into the admin bar.

 There are several known bugs:
 1. Screen Options and Help cannot be expanded simultaneously.
 2. The implementation of Search is really hacky, and will break on non-
 English installations.

 Both should be fixed before going into core.

 Again, this is a first pass. Things will change. Freeze is fast
 approaching and we're not quite a quiet bunch, so let's get the ball
 rolling.
