 The default 'Name', 'Slug' and 'Description' form fields are stuck in
 place on Categories and Custom Taxonomies.
 There are hooks before and after the form (eg {$taxonomy}_pre_edit_form),
 but nothing to modify the basic output.
 These fields, especially 'slug', can scare civilians and it would be nice
 not to have to use JS to hide them.
