Comment:

 I have tested, but cannot replicate this issue.  I have tested on both 3.1
 and 3.2-bleeding.  I tried using default permalinks and month and name.

 Just for kicks have you tried not using {{{$_SERVER['DOCUMENT_ROOT']}}}
 but actually using the real file path?  I don't see how that could effect
 anything, but worth trying.
