 Can't reproduce.

 This is how it goes for me:

 1. Activate plugin.
 2. Access wp-admin/options-permalink.php
 3. Go to http://localhost/wp/book/test-book

 No 404s. Have you tried deactivating all other plugins and switching to
 the default theme?
