 1. I get both sets of icons on the toolbar - both the visual and the HTML
 - see a screenshot here -

 2. When entering links in visual I get glitchy visual errors - doubled up
 X button, weird white opacity strip -

 When I click Update, I get taken straight back to the post list, link
 creation having failed. The only way to make links is to do it in code.
