 When I try to add custom buttons to the tinymce editor, as stated here:
 [http://codex.wordpress.org/TinyMCE_Custom_Buttons], every time I edit a
 post (with the visual editor as the default editor) the HTML editor's
 buttons are displayed on top of the visual editor's. Screenshot:

 This happens on a clean WP 3.0.4 installation. I'm using Firefox 3.6.1.3.
