Comment:

 If the new custom permalink structure is still "/%postname%/" then the
 radio button for "Post name" will continue to be checked. This is correct.

 If I change my permalink structure from "Month and name" to a really
 custom structure, like "/%postname%.html" the actual structure is changed
 and he radio button for "Custom" is checked, showing the selected pattern.

 Suggest worksforme
