 This comes to light with recent Twitter changes to how tweet permalinks
 work. Example: http://twitter.com/#!/wordpress/status/25907440233

 When filtered with {{{make_clickable}}} the URL result stops at the #
 character. Resulting HTML:

 {{{
 <a href="http://twitter.com/#" rel="noreferrer
 nofollow">http://twitter.com/#</a>
 !/wordpress/status/25907440233
 }}}

 Adding ! to the pattern would avoid this.
