The current system 'allows' for overriding of items, that is to say if you
specify a $position that already exists it will simply overwrite it. I'm
not sure if this was intentional or not but it seems like strange
behaviour to me. I can see that you might wish to allow people to override
items, but it just seems to easy to do by accident. I'd personally like to
see this as an option, but by default shift items down out of the way.
($position_override=FALSE for example).
