 get_the_post_thumbnail() calls wp_get_attachment_image() with a 4th non-
 existant $attr argument.

 I am not sure whethere $attr should be removed from
 get_the_post_thumbnail() as it is only used in the return filter (maybe if
 we remove it we will loose backward compatibility)
