 Thinking more about this: best "fix" is when the site has an actual icon.
 Don't think WordPress should be substituting it if it doesn't exist
 (millions and millions of web pages with the same favicon).

 Also the browsers seem to have "special" caching for these icons, don't
 think we need to try to refresh them every few hours (would probably
 affect only network caches).

 3426-4.patch is based on 3426_favicon_v2.diff.
