 Salts and Keys were added to MU at various points between 2.6 & 2.8. Every
 time one (or more) salt/key was added, some people reported this problem.
 Clearing cookies and/or browser cache has worked since 2.6. This isn't a
 problem with WordPress. For whatever reason (quite possibly a security
 measure), in some instances, the browser will not update the existing
 cookie with a new one.

 The best we can do here is add information to the UI and/or the codex.
