 In the attached patch I created a new method in wpdb called
 _insert_replace_helper, which is meant to be a private method.  Both
 wpdb::insert and the new wpdb::replace use it.  If whomever is in charge
 prefers it to be done another way, just let me know and I'd be happy to
 make a new patch.
