Comment:

 Still happens when all plugins are disabled. I've had this problem with
 two different sites. I've also had the problem with WP 2.4. I've seen some
 people complain of similar problems with the image uploader recently. They
 said that sometimes the image would show up, but other times it wouldn't.
 I believe that was on the #wordpress channel of the IRC.
