 Yep, The blank-item was something i mentioned in my attachment comments.

 It'd happen with the category as well if it wasn't for the fact of the
 default category.

 Another option might be to automatically create the %taxonomy-query-arg%
 when new taxonomies are added.. but that'd need back-compat for categories
 and possibly tags.. and possibly change the entire way the
 WP_rewrite/WP_query classes operate..
