 Most people don't use dedicated hosts, they use shared ones.

 And I would debate the accuracy of that codex page. While I've not used
 most of those hosts, I have seen setups on many of them. BlueHost, to pick
 the first example, definitely does NOT use "localhost" on their shared
 hosting services.
