Comment:

 This doesn't fix the problem when a plugin uses dbdelta() like [http://dev
 .wp-plugins.org/file/gengo/trunk/gengo/gengo_schema.php Gengo] does.  The
 UNIQUE index on the code field will be duplicated.

 ryanscheuermann's patch fixed _that_ problem for me.
