#6030: Track pull requests on my-patches page

As today, the https://core.trac.wordpress.org/my-patches shows the tickets
where you have contributed with a patch, but this doesn't consider the GH
pull requests you did that after all patches and contributions to a
 ticket.
So you cannot track those inside trac, but you need github with a URL like
 https://github.com/WordPress/wordpress-
 develop/pulls?q=is%3Apr+is%3Aopen+sort%3Aupdated-desc+author%3AMte90
 (change the author name).

The only solution is to export a diff and upload it in the ticket, but it
 isn't a scalable solution.

I think that is can be related to
 https://meta.trac.wordpress.org/ticket/4447 and
 https://meta.trac.wordpress.org/ticket/5592
