 When a user's role is changed via editing their profile, we need to log
 the action (who did it and when).

 This would be for all state changes (change someone to a mod, block a
 user, etc)

 Also we need https://meta.trac.wordpress.org/ticket/4192 so that if you
 change the state, it's '''required''' to leave a reason why.
