 Some minor suggestions, all IMHO of course! :)

 1. Documentation doesn't "work" :) Maybe "**Did you find what you were
 looking for?** "
 2. Second sentence is incomplete. Try "**Is something missing, or off?**"
 3. Third sentence implies that the reporter had noticed there was no way
 to submit issues previously. Is this the case? Regardless, saying so
 doesn't add anything meaningful; Try simplifying to "**Let us know right
 here.**"
 4. "**Comments you send to us will go...**" is a bit more friendly
 sounding to me.
 5. "Surface" is lingo. Try "Raise" or "Found," like "**The issue you found
 will...**"
 6. We dropped the website, so drop that from the Save checkbox too :)
