 The Polyglots teams page - https://make.wordpress.org/polyglots/teams/ -
 can be filtered to return team-specific information. E.g.,
 https://make.wordpress.org/polyglots/teams/?locale=id_ID.

 However, the title tag and canonical URL tag don't update accordingly.

 * The title tag should be altered from `Translation Teams – Translate
 WordPress` to `Translation Teams – {{LOCALE}} – Translate WordPress`.
 E.g., `Translation Teams - Portuguese (Brazil) - Translate WordPress`.

 * The canonical URL tag should be updated to reference the filtered URL
 state.

 Ideally, the URL structure would also be 'upgraded' to use a clean format,
 such as https://make.wordpress.org/polyglots/teams/pt_BR/, or
 https://make.wordpress.org/polyglots/teams/locale/pt_BR/
