 Comment authors should get an email if their comment is deleted or
 approved. This adds transparency to why (and when) an admin has approved
 or deleted their comment. A simple thank you email could be send if a
 comment is approved.

 When deleting a comment the default email text could outline the possible
 reasons why it was deleted.

 * comment issue is resolved and is no longer relevant to the documentation
 * comment is a support question. (please open a forum topic)
 * comment is not adding to the documentation or is a duplicate comment.
 * comment code contains errors or is broken
 * comment code not adhering to code standards
 * comment contains self-promotion or spam

 Admins should be able to edit the default email text(s) when they approve
 or delete a note.
 Comment authors should be able to opt out of these emails in their profile
 page.
