New description:
I try to search tickets that contain 2 focuses e.g. "accessibility" and
"meta".
From the Make WordPress Accessible you are linked to
https://core.trac.wordpress.org/query?status=!closed&focuses=~accessibility
Here I want to add the focus "meta", so I go to the "And" pulldown, than
"focuses". A second line appears, but with the word "or" in front of it.
When executing the query, the result is "or".
Work around is to add the second search phrase in the first text box.
Basically the And-pulldown doesn't as expected.
On https://trac.edgewall.org/query this issue is taking place as well.
So it is principally an issue for trac, but there might be a WordPress
solution.

FWIW, there's no `meta` focus on WordPress Core Trac. There is a
[https://core.trac.wordpress.org/query?status=!closed&milestone=WordPress.org
WordPress.org milestone], but all new tickets for the WordPress.org site
are created here, on Meta Trac.
