 In a similar vein to #363, it would be useful to allow plugin/theme
 authors to provide a support template for users to submit their support
 requests based on pre-given questions.

 Github implemented it this way:
 https://github.com/blog/2111-issue-and-pull-request-templates

 Potentially, a plugin author could include a file in their repo called
 .support to pre-populate the New Ticket field. It's unclear to me the
 complexity of this request but it would certainly make the back and forth
 of support tickets in the Support Forum in general much more efficient if
 well-implemented.
