#2518: Replies can disappear for a short time for no obvious reason

Stumbled upon this several times in the last few months.

Occasionally, when replying to a topic, your reply is not displayed right
away. It's not in pending or spam queue, apparently it just falls through
the cracks of cache buckets :)

The reply itself might appear after a few page refreshes, but the Last
Post info still shows the previous poster's name even after a few hours
(this is ''not'' related to #2043, as no pending, spam, or archived posts
are involved), and I suspect the notification for the reply didn't go out
 either.

The workaround I use is to unapprove and re-approve my own post, then the
Last Post info is properly refreshed.

Is there any other info that might be helpful in tracking this down?
