 * Preview is fetched via AJAX.
 * Preview updates 1.5 seconds after user stops typing.
 * Adds editor toolbar button "preview note".
 * Moves filtering of 'syntaxhighlighter_htmlresult' outside of just
 handbooks since it can also be applied elsewhere.
