What about using a simple Google Docs Form and integrating it with your
site?  You can very quickly create a form in Google Docs and all the
responses end up in a spreadsheet which is pretty easy to export to
something else.  You can use (shameless plug) the WordPress Google Forms
plugin to put the Google Form on a page or post on your WordPress site.

We do something very similar to what you are describing with our Middle
School Booster Club and using a Google Form is how I solved the problem.
