Not sure about your icon question but its probably covered in the codex
article or the linked TMCE reference articles.

Here's a WP Ideas forum suggestion to make an easy api for adding buttons,
which should make the button image a single field:
(the order of the posts seems to have gotten out of whack in some recent
updates, but IMHO the thread still makes sense)
