[theme-reviewers] Accessibility Guidelines / Buttons

Getting more examples in place for the accessibility review is one of my
goals for the next few weeks. There should be examples available for all of
the requirements, to give people a good idea of what kinds of patterns to
follow.
I'll be working on a draft revision to this document in the next few weeks;
I want to reorganize it a little bit, as well.
